Summary of Position
Serves as a primary health care provider within the scope of practice according to the requirements of the Ohio Board of Nursing/Ohio Medical Board, written protocol, and ability.

Responsibilities and Duties:
1. Embraces an active and healthy lifestyle including smoke-free living, stress reduction, exercise, work/life balance and healthy dietary choices. 2. Participates in and promotes health and wellness activities within the organization.
3. Supports a Patient Centered Medical Home (PCMH) where the patient-provider relationship is encouraged, with an emphasis on coordination of care and the establishment of a long-term healing relationship.
4. Supports open patient access to and coordination of care that is supported by information technology.
5. Promotes effective and appropriate use of integrated health services as part of a “Patient Centered Medical Home” (PCMH) model of care.
6. Serve as a member of the Fairfield Community Health Center (FCHC) health care team that provides quality care to patients.
7. Maintains a good working relationship with collaborating physician and other team members.
8. Coordinates patient services and institute referrals to promote and maintain continuous comprehensive care at every level.
9. Incorporates preventive health care provisions aimed at improving and maintaining health status based upon individual, cultural, sociological, psychological, and environmental factors.
10. Provides precise information to enable the patient to make informed decisions regarding health care and related issues while ensuring joint responsibility with the health care team members and staff to promote the best outcome possible for the patient.
11. Advises and counsels patients about normal maturational and developmental stages and natural life cycles. Targets any problem areas, intervenes, and participates in problem solving.
12. Assesses and intervenes in ordinary developmental and behavioral problems of individuals and their families.
13. Collects and constructs identifying information by reviewing past and present medical, social, and family histories, performing a physical examination, assembling a problem list, documenting, and consulting with the Chief Medical Officer in changes or progression of chronic disease.
14. Performs routine physical examinations and preventive health measures including, but not limited to immunizations, developmental screening, routine laboratory screening/procedures appropriate to each gender and age group.
15. Diagnoses and treats common and uncomplicated health conditions. Collaborates, consults, and refers any patients who have complicated or uncommon health problems to other physicians in the health center or the community.
16. Provides education about diagnosis to the patient and provides written follow-up guidelines. Includes family members according to patient wishes. Physician’s Assistant Position Description 2
17. Charts in the Electronic Medical Record (EMR) utilizing age appropriate wellness assessment tools or problem oriented medical record system, assuring accurate and timely completion of data entry into the patient medical record.
18. Provides support and counseling for patients with stable chronic diseases in regard to physical, psychosocial, and environmental well-being. Refers any patient with unstable chronic disease to other physicians at the health center.
19. Provides contraceptive counseling to men and women who seek birth control, making available the desired birth control measures when possible or refer to agency where desired birth control measures may be obtained.
20. Prescribes drugs and devices incidental to the provision of routine health care, family planning, and perinatal care for essentially healthy patients in accordance with State of Ohio law.
21. Institutes emergency medical measures in life-threatening situations. Provides first aid as needed.
22. Follows universal precautions.
23. Utilizes supplies in a cost-effective manner.
24. Performs clinic and outreach related tasks.
25. Utilizes on-going assessment skills.
26. Participates in Quality Improvement and PCMH activities.
27. Demonstrates effective communication skills.
28. Establishes and maintains professional working relationships with other members of the health care team.
29. Participates in planning and problem solving.
30. Participates in continuing education and professional growth.
31. Attends staff meetings.
32. Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures.
33. Assists with seasonal programs as needed.
34. Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices. 35. Performs other duties as assigned.

Working Conditions/Environment:
Prolonged sitting or standing may be required. Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with duties in a clinic office setting will be performed on a repetitive basis. This also includes bending, twisting, reaching, lifting, pulling, pushing, and walking. The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals, fax machines, and copiers is required. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is frequently exposed to communicable diseases, toxic substances, medicinal preparations and other conditions common to a clinic environment. The noise level in the work environment is usually moderate. Working under stressful conditions as well as irregular hours may be required.

Qualifications:
1. Master of Science Degree in Nursing/Physician’s Assistant.
2. Current license to practice as an Advanced Practice Nurse or Physician’s Assistant in the State of Ohio.
3. DEA license in good standing.
4. BLS (basic life support)/CPR (Cardiopulmonary resuscitation) required.
5. Ability to work as a team member.
6. Computer experience desirable.
7. Ability to work with a culturally diverse group of people.
8. Must be willing to work flexible hours as required by position.
