Summary of Position
This is a salaried and 1.0 FTE position working under the supervision of the Chief Medical Officer with a signed Standard of Care Arrangement for Advanced Practice Nursing. The employee is assigned to a collaborating physician in accordance with Ohio State Nursing Board requirements and the employee works closely with physicians and other clinicians in a team approach to patient care.The employee is responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients.

Principal Duties and Responsibilities

Patient/Customer Focus: Ensures that staff members make patients and their needs a primary focus of their actions; showing interest in and understanding of the needs and expectations of internal and external customers; gaining patient trust and respect; meeting or exceeding patient’s expectations.   • Assesses patient status by obtaining health history through patient/family interviews and chart reviews.
  • Assesses present and past illnesses, risk factors and family history, psychosocial situation, and cultural factors.
  •  Performs appropriate physical examinations.
  • Orders/performs appropriate laboratory, diagnostic, and other screening tests.
  • Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illnesses.
  • Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.
  • Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.
  • Handles minor lacerations.
  • Complies with patient confidentiality requirements and promotes patient advocacy.
Quality Orientation: Monitors and ensures that quality standards are being met at each office. Provides guidance to ensure that quality levels of care are provided and that documentation of the same is thorough, complete, and accurate. 
  • Assists with the timely implementation of Quality initiatives, regulatory compliance programs (BPHS-330 Health Plan), and process improvement projects.
  • Assists the office in creating and monitoring action plans resulting from patient and employee satisfaction surveys.
  • Participates in peer reviews, chart review and audits, staff education, clinical guideline development, and other continuing education and quality improvement activities to demonstrate compliance with standards, regulations, policies, and procedures.
  • Assists the CMO, Center Physician(s), FCHC CMO, and the FCHC QI Coordinator in developing, implementing, and evaluating the Quality Improvement (QI) Program at the main office and the off-site facilities.
  • May be asked to serve on the FCHC QI Committee.
Technical/Professional Knowledge and Skills: Possesses, acquires and maintains the technical/ professional expertise required to do the job effectively. Demonstrates knowledge through problem solving, applying professional judgment, and competent performance. 
  • Knowledge of the advanced practice of nursing, including anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, and alternative treatment modalities.
  • Knowledge of professional nursing theory, practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery of nursing care.
  • Knowledge of medical equipment and instruments used to administer patient care.
  • Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
  • Knowledge of community, professional, and educational resources available to professionally treat the whole patient.
Planning and Organizing: Organizes work, sets priorities, and determines resource requirements; determines necessary sequence of activities needed to achieve goals. 
  • Skill in case management, time management, problem solving, crisis intervention, multitasking, and other organizational matters.
  • Skill in modifying methods and techniques of professional nursing to include appropriate nursing care and in initiating appropriate emergency response and investigatory procedures.
  • Skill in using computerized Practice Management Systems (PMS) and Electronic Medical Records (EMR) when such systems become fully operational.
  • Ability to analyze complex data and draw conclusions.
  • Ability to interpret, adapt, and apply guidelines, policies, and procedures.

Minimum Qualifications 
  • AANP or ANCC certification as a Nurse Practitioner is required.
  • Must be licensed as a Registered Nurse to practice in Ohio, pursuant to ORC 4723.12.
  • Must have a Master of Science in Nursing.
  • Three to five years medical supervisory experience is preferred.
  • Excellent written and oral communication skills are preferred.
  • Excellent computer skills are preferred.
  • Must be licensed to drive in the state of Ohio and have reliable transportation.
  • Must be willing and able to occasionally travel to other sites in addition to attending out-of-town meetings.

Unusual Working Conditions/Hazards
The position requires the employee to respond to emergencies at any hour of the day or night, with the potential to work longer than normal hours. Physical effort includes normal bending, lifting, walking, standing and sitting inherent to a busy health center.
