FAIRHOPE Hospice & Palliative Care, Home of The Pickering House, is seeking an experienced customer service and skilled clerical candidate to serve as a Palliative Navigator for its Palliative Care Program and the FAIRHOPE Advanced Illness Management Clinic. The ideal candidate will be highly organized, flexible with changing needs, and have excellent communication skills.

The successful candidate will be responsible for supporting day-to-day workflow operations, performing comprehensive administrative and clinical support tasks, and ensuring high-quality customer service for the FAIRHOPE Palliative Care Program and FAIRHOPE’s Advanced Illness Management Clinic. They will also serve as a liaison between departmental teams and interact with customers. This position requires applicants to possess extensive computer skills, ability to maintain confidentiality, and use initiative to resolve challenging tasks.

Responsibilities:

  • Actively listen, document, and facilitate all inquiries and referrals for Palliative Care program patient services in collaboration with FAIRHOPE Admission Navigators and the FAIRHOPE Palliative Care program team.
  • Accurately document relevant clinical notes regarding phone calls, office visits and care provided. Triage telephone calls and disperse to appropriate clinical and/or on-call staff.
  • Coordinate and track all Palliative Care Program referrals, admissions, and required patient visits with clinical staff as needed.
    • Facilitate admission paperwork and gather patient demographics, history, physical data, and any additional pertinent information to initiate care as needed.
    • Receive documents and facilitate all inquiries and referrals for patient services in collaboration with the Admissions Navigator.
  • Work with FAIRHOPE Billing team to verify coverage and obtain precertification from insurance companies as needed.
  • Communicate with health care providers, hospitals, home health agencies, and physicians to provide information on available services and obtain information for patient referrals.
  • Schedule patient admissions and visits for the FAIRHOPE Advanced Illness Management Clinic and/or patient's home setting.
  • Prepare patient rooms for clinic visits.
  • Perform Palliative Care Program telephone customer service calls to new admissions as needed and follow-up calls to patients from previous evening and/or weekend on-call visits.
  • Maintain clinic supply stock inventories.
    • Organize supply rooms, maintain cleanliness, and ensure orderliness in the supply room.
    • Order supplies as needed, utilizing FAIRHOPE’s ordering process.
    • Maintain Safety Data sheets associated with chemical products within the clinic.
  • Assist with the following clinical support functions: faxing, preparing clinical documentation for filing, and copying. File all documentation pertaining to clinical records and addresses problems/concerns regarding medical records.
  • Process FAIRHOPE Advanced Illness Management clinic agency interoffice and USPS mail as needed.
  • Electronically document any direct services provided and ongoing communication with patients, families, and community providers.
  • Remain HIPAA compliant at all times.

Physical Setting: In-office environment. Driving may be required.

Work Location: Offices located in Lancaster, Ohio. Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties.

Qualifications:

  • Certification or Registration in medical assisting or a Licensed Practical Nurse in the State of Ohio.
  • Associate’s Degree in Administrative Studies or Business Administration or high school diploma with a minimum of 3 years clinic or physician office experience.
  • Excellent computer skills with work experience and advanced knowledge of Microsoft Office products.
  • May require evenings and weekends as needed to assist with events or meetings. Also, it may require traveling to other sites as needed. Travel may be in inclement weather.
  • Ability to manage time efficiently.
  • Must have and maintain a valid driver’s license, good driving record, and auto accident insurance at a level acceptable to FAIRHOPE and/or FAIRHOPE’s insurer.
