Hospice Social Worker

FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a full-time Hospice Social Worker to help patients and families navigate planning for end-of-life care. Helping patients, families and caregivers understand their treatment plan and manage the stresses of debilitating physical illnesses, including emotional, familial and financial.

We believe in a professional team approach and candidates must demonstrate great communication and social interaction skills, as well as, be a compassionate individual to work with patients, families and other professionals to promote the philosophy of hospice and palliative care during an often stressful, challenging and difficult time of their lives.

FAIRHOPE relies on its Hospice Social Worker to be an advocate for patients and their families, knowing what their patients need and what resources are available to them within and outside of Hospice settings to help.   • Responsibilities:
  • Assess the psychosocial status of the patient/caregiver/family related to the Palliative Care patient’s terminal illness and the patient’s environment to identify any areas of safety or concern.
  • Carry out social evaluations and plan intervention based on evaluation findings.
  • Counsel patient/family/caregivers as needed in relationship to stress, and other identified coping difficulties.
  • Discharge planning. Assist patient/caregiver/family with securing advance directives and funeral arrangements, as needed.
  • Serve as liaison and support to public and private agencies as resources for patient and personnel.
  • Document electronically direct services provided and ongoing communication with patients, families and community providers.
  • Remain HIPAA compliant at all times.
Qualifications:
  • LSW or MSW degree and current valid Social Work licensure in the state of Ohio; MSW preferred.
  • A minimum of one year's experience working in healthcare is required and end-of-life care and/or hospice and palliative care experience is preferred.
  • Ability to analyze and implement proper action is essential. Demonstrate great communication and social interaction skills, as well as be a compassionate individual.
  • Computer skills required.
  • Physical Setting: Various healthcare settings. Some driving is required.
  • Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.
