Join the Fairfield County Board of Developmental Disabilities as an Individual Support Coordinator and play a vital role in empowering individuals to live self-directed, meaningful lives. In this position, you will coordinate person-centered services for individuals with developmental disabilities, guiding assessment, planning, and implementation of supports that reflect each person’s strengths, goals, and preferences. You’ll collaborate with individuals, families, providers, and community partners to develop and monitor service plans and budgets, ensure quality care, respond to crises, and connect individuals to resources that enhance independence and well-being. This role also includes on-call responsibilities and ongoing documentation to ensure compliance and continuity of care.
We’re seeking a compassionate, organized professional with a bachelor’s degree and eligibility for DODD certification, along with a valid Ohio driver’s license and insurable driving record. The ideal candidate demonstrates strong communication, sound judgment, and the ability to work both independently and collaboratively. A commitment to dignity, respect, and advocacy for people with disabilities is essential, as is basic computer proficiency.
