The Fairfield County Board of Developmental Disabilities is seeking an Individual Support Coordination Supervisor. This position is responsible for support, direction, supervision, and training to the Individual Service Coordinators (ISCs) and Service & Supports Assistants. Assures Fairfield DD personnel policies and procedures are followed. Recommends hiring, firing and disciplinary action to the Director of Service and Supports. Completes evaluations of staff annually. Meets with staff quarterly to review progress toward performance goals.

The starting annual rate for this position is $61,588.80 and is based upon experience and qualifications and includes full county benefits. A Bachelor's Degree or higher plus minimum of three years of managing staff in a professional environment, a minimum of one year working as an Individual Support Coordinator for Fairfield DD, eligible for required DODD certification, knowledge of Medicaid and insurance billing procedures, and maintains a valid Ohio driver’s license and a driving record acceptable to agency insurance carrier. SERVICES AND SUPPORTS SUPERVISOR
REGULAR HOURS OF WORK: 8:00 a.m. to 5:00 p.m. M-F (may be required to work a flexible schedule)
IMMEDIATE SUPERVISOR: Director of Services and Supports
DEPARTMENT: Services and Supports
SPECIFICATIONS:
As an employee of the Fairfield County Board of Developmental Disabilities, the job incumbent shall comply with all Board policies at all times, and shall demonstrate respect for, support dignity of, and observe the rights of all individuals served by the agency.
DUTIES:
30% Management
Assures Fairfield DD personnel policies and procedures are followed.
Provides interpretation and guidance to ISCs on policies, procedures and rules related to their job, specifically Ohio Revised Code 5123-4-02 Service and support administration.
Plans, schedules, and conducts quarterly team meetings to address department and agency needs and changes. Meets with employees quarterly to review progress towards performance goals.
Provides support, direction, supervision, and training to the Individual Service Coordinators (ISCs) and ISC Assistants. Assures Fairfield DD personnel policies and procedures are followed. Recommends hiring, firing and disciplinary action to the Director of Service and Supports. Completes evaluations of staff annually. Meets with staff quarterly to review progress toward performance goals.
Provides interpretation and guidance to ISCs on policies, procedures and rules related to their job, specifically Ohio Revised Code 5123-4-02 Service and support administration. Plans, schedules, and conducts monthly ISC meetings and team meetings, as needed to facilitate skill and team building.
Reviews and approves Time Clock within required timeframes. Approves leave requests. Tracks information for trends and patterns. Ensures adequate office coverage. Supports team and individuals receiving services when the ISC is on leave/or unavailable.
Provides supervision and support to ISCs on-call.
Monitors Targeted Case Management (TCM) documentation along with recording of non-TCM activities, general administrative time (GAT), travel and documentation time. Reviews and signs all documents as required for non-certified staff. Conducts regularly scheduled and as needed case note reviews to ensure quality, accuracy, and timeliness of documentation. Reviews and tracks productivity for ISCs that is based on hours worked and billable units. As needed, strategizes with ISC’s on ways to increase productivity to meet and/or exceed standards.
If applicable, monitors authorizations and management of FSS and summer scholarship applications.
25% Individual Support Coordination Supervision
Reviews and approves Individual Service Plans (ISP). Sends service summaries to Resource Management team for processing. Ensures an individual’s ISP contains all required elements including a thorough assessment of the individual and measurable outcomes. Tracks information related to timely completion of ISPs. Proactive trend analysis with the monitoring of the timely completion of individual service plans.
If applicable, directs and manages the functions of introduction and eligibility for Board services and supports. Develops and maintains information within the main information system database of individuals eligible and ineligible.
Works with Introduction and Eligibility Specialist to assign individuals to ISCs. Ensure ISCs maintain a balanced caseload of individuals supported.
Empowers teams to identify and address immediate and current needs of the individual. Assists as needed with the implementation of the waiting list assessment tool. Guides a person’s ISC and team through the request for local funding or waiver funding as determined appropriate by the tool and identified needs.
Promotes the principles of self-determination and person-centered thinking and planning. Encourages individuals to establish and achieve outcomes related to independence, personal growth in community engagement, employment, and other life areas. Ensures a continuous review of progress towards outcome is occurring. Monitor the budget and ensure direction and activities work in conjunction with the Mission of FCBDD.
Meets regularly with the Director of Services and Supports to discuss trends, patterns, and needs. Brings critical issues regarding individuals and families as well as suggestions for policy direction to the attention of the Director of Services and Supports. Provides input to the monthly Board report.
Maintains all required documentation of activities.
Works to improve and enhance the quality and appropriateness of services and supports rendered to individuals. Reviews Person Centered reports or Quality Assurance reports from contractor. Reviews ISPs for Quality and to ensure important elements or requirements are contained within plans.
Works with planning teams to develop resources and supports for plan implementation. Monitors the implementation of the planning process to ensure the desired outcomes. Provides leadership in implementing services.
If applicable, monitors the Introduction and Eligibility activities and makes recommendations to the SSA Department regarding capacity and staffing needs related to trends in people served.
If applicable, Conduct eligibility assessments for appeals.
10% Communications and Training
If applicable, coordinates the Transition Partnership Collaborative group, setting agendas, notifying stakeholders, sending minutes of meetings, etc. Serves as liaison for collaborative transition projects on behalf of the FCBDD. Participates in Transition Collaborative Partnership projects related to transition. Serves as a consultant and trainer to classroom teachers and other educational personnel/support staff/volunteers in school programs and vocational training sites throughout the community in areas specific to transition programming and future planning.
Makes contacts in the community to develop additional resources for assigned individuals.
Communicates with the agency regarding issues involving services. Facilitates communication between Board, schools, other agencies, and families.
Provides presentations and information concerning the Board, department, and work unit in accordance with Board philosophy and mission. May be asked to provide/participate in activities internal and external to the agency. Participates in and provides in-service training as may be required.
Collaborates and coordinates with the Professional Development Coordinator to ensure self-determination and transition training occurs on an annual basis to enhance transition outcomes through self-determination.
Serves as a liaison by communicating with all parties, including individuals, family members, chosen representative, guardians, other team members, providers, other County Board staff members and regulators.
Maintains contact with Early Childhood, School Age and other agency staff and contract providers regarding services provided.
Contributes to FCBDD website regarding services, as requested.
Develops written materials to assist others in understanding processes, as requested.
10% Collect, Maintain and Interpret Data
Reviews incident reports and outcome monitoring forms as necessary.
May serve as member of human rights or other committees internal and external to the agency to fulfill the agency’s mission. Participates in development and implementation of quality improvement plan.
Identifies data trends and patterns and draws conclusions from survey results.
Prepares and maintains required reports, records, correspondence, and other communication as necessary.
If applicable, collects and reports upon data related to eligibility trends and patterns along with transfers in/out of the county for services.
If applicable, monitors the benefit and housing coordination activities trends and patterns of those served.
If applicable, oversees public housing benefits through Fairfield Metropolitan Housing Authority (FMHA) and exploring other community assistance funding available.
If applicable, oversees SSA quality assurance functions which include: accreditation, provider reviews, quality assurance reviews of ISPs, and RNQA assessments to determine trends, patterns, and best practices.
10% Assists the Director of Service and Supports
Meets regularly with the Director of Services and Supports to discuss trends, patterns, and needs. Brings critical issues regarding individuals and families as well as suggestions for policy direction to the attention of the Director of Services and Supports.
Assists the Director of Service and Supports to assure dissemination of information to staff, individuals, parents/guardians, caregivers, and providers.
Works with the Director of Services and Supports to assure the continued development and implementation of the agency’s strategic direction, including the individual planning process, person-centered planning, and individual budgets.
Assists the Director of Service and Supports to further develop systems and procedures to comply with applicable laws, rules, and regulations as well as Fairfield DD Strategic Plan.
Provides input to the monthly Board report. Writes a monthly board report sending it to the Director of Services and Supports for inclusion in the board packet and to inform other parties as needed. If applicable, monitor and report to the Board on a monthly basis the number of individuals who are and are not eligible for services, trends for self-advocacy, and growth in a community job or volunteer experience.
Represents the Director of Service and Supports as assigned.
10% Communications & Community Relations
Provides consultative and technical assistance to partner agencies.
Serves as a liaison between Fairfield DD, Ohio Department of Developmental Disabilities (DODD), families, providers, school districts and other agencies regarding Services and Supports.
Maintains and provides information related to Social Security benefits, employment supports, Medicaid, supported decision-making, community social services and other resources.
5% Other
Furthers the mission of the Fairfield DD by interacting in a courteous and professional manner with individuals receiving supports, their families, and friends and with co-workers, supervisors, and members of the community.
Maintains confidentiality of records, information, and program matters.
As necessary complies with Safety Rules established for the purpose of fulfilling compliance with Ohio Employment Risk Reduction Standards, Rules, or Orders, through the utilization of protective equipment and decontamination techniques.
Performs other duties as director by the Director of Service and Supports and/or Administration.
QUALIFICATIONS:
- Bachelor's Degree or higher plus minimum of three years experience working with persons with developmental disabilities.
- A minimum of one year working as an Individual Support Coordinator for Fairfield DD
- Eligible for required DODD certification (Service and Support Administration Supervisor)
- Successful completion of Fairfield DD’s Emerging Leader Program and NACo academy.
- A minimum of three years managing staff in a professional environment.
- Proven track record of leadership at Fairfield DD.
- Knowledge of Medicaid and insurance billing procedures
- Maintains a valid Ohio driver's license and a driving record acceptable to agency insurance carrier.
- May be requested to work flexible hours based on service needs.
